Hello, my name is Madison Francis and I am your new school nurse. My husband, Trevor, and our two girls, Ellison and Rubie, keep me busy when school is not in session. My hobbies include watching sports, cheering on Ellison and the panthers. I enjoy playing softball, reading, camping, and spending time with friends and family. I completed my BSN through Ohio University and have worked for our local health system until now.
I am sending this letter before school starts to provide education and expectations. If your child/children will be taking prescription medications during school hours, we must have all appropriate forms filled out before administration of medications. These forms (Medication Request and Medication Administration Record) can be found under "Forms and Links" on the schools website. Please send the form provided to your child/children(s) physician to have all the proper paperwork completed. After this is completed and the form is received, I will need a parent or guardian to bring in the medication in the proper bottle/box with the most current prescription label. At school, we will count the medication together and I will need a signature on the amount of medication(s) provided. All medications must be registered with the Nurse’s Office where I will have a record of medication administration, dosage, date and time.
